Cardiac Recovery Services

In addition to Touchpoints’ signature Congestive Heart Failure rehabilitation program, we also offer cardiac recovery services for those with recent heart surgery or other heart related issues requiring rehab. In order to get you back on your feet and back home where you truly want to be, we offer the following services:

• Ongoing physician/cardiac APRN evaluations
• Cardiac education for you and your family
• Heart healthy menu
• Weight monitoring—Receive your own scale and weight log to take home with you
• Physical, occupational and speech therapies—up to seven days a week
• Customized care planning
• Home support and discharge planning
• Consulting Cardiologist on staff
